Here is the intro for the survey:

This survey is being conducted as part of a doctoral course (SAER 8321 – Survey Methods in Educational Research) at the University of Houston’s College of Education.

The survey attempts to identify librarians’ attitudes towards instructional design and multimedia technology, including librarians’ own beliefs about their abilities to utilize multimedia technology in the classroom.

All survey responses are confidential and no personally-identifying information is collected.

After May 1, 2006, results of the survey will be available at: http://www.hilyer.info/uh/survey.html
